Position Overview
We are seeking a dedicated and client-focused Rehabilitation Counselor to join our team. The Rehabilitation Counselor will support individuals facing physical, emotional, mental, or vocational challenges by assessing needs, developing individualized rehabilitation plans, and providing ongoing counseling and case management. The ideal candidate will be skilled at motivating clients, coordinating resources, and creating pathways toward increased independence and quality of life.
Key Responsibilities
- Conduct comprehensive assessments to identify clients’ strengths, limitations, vocational goals, and rehabilitation needs.
- Develop and implement individualized rehabilitation or treatment plans tailored to each client.
- Provide counseling services to help clients manage emotional, behavioral, or vocational challenges.
- Assist clients in developing life skills, coping strategies, and self-advocacy abilities.
- Coordinate services with medical providers, therapists, employers, and community organizations.
- Support clients in accessing resources such as housing, education programs, job training, and employment opportunities.
- Monitor client progress and update rehabilitation plans as needed.
- Maintain accurate and timely documentation in compliance with company policies and state regulations.
- Provide crisis intervention and risk assessments when necessary.
- Participate in team meetings, case consultations, and ongoing professional development.
Qualifications
- Master’s degree in Rehabilitation Counseling, Counseling, Psychology, Social Work, or a related field.
- State licensure or certification as a Rehabilitation Counselor (e.g., CRC) preferred or the ability to obtain it.
- Experience working with individuals with disabilities, mental health challenges, or vocational needs.
- Strong knowledge of rehabilitation techniques, resources, and community support systems.
- Excellent communication, problem-solving, and case management skills.
- Ability to work independently and collaboratively.
- Proficiency with electronic health records (EHR) is a plus.
Preferred Qualifications
- Experience in vocational rehabilitation, disability services, or community mental health.
- Training in evidence-based counseling techniques.
- Bilingual skills are a plus.
